You will enter this as a  W-2 

OMB No. 1545-008 is the government official number for the W-2 Form 

Look to the right of box a and you will see this number on page 2 of this link https://www.irs.gov/pub/irs-pdf/fw2.pdf

To enter a W-2 -

• Click on Federal Taxes 
• Click on Wages and Income 
• Click on I'll choose what I work on
• On Wages and Salaries, click the start or update button
