Why am I being asked to fill out a Schedule 4 ?

Without access to your tax return, I can't saw for certain.

Schedule 4 reports other taxes such as self employment and nanny taxes.

 It also reports the penalty for failing to have health insurance. This is the most likely cause of the need for the form.

If you don't want to pay the upgrade fee, I suggest TurboTax Free File Edition. 

While the name is similar to Free Edition, there are no upgrade fees.

To you qualify

Make less than $34,000 a year or

Qualify for the Earned Income Credit or

Active military making less than $66,000
