Make your check payable to "Idaho State Tax Commission and mail your Idaho state tax payment to:

 Idaho State Tax Commission 

 PO Box 83784

 Boise ID 83707-3784

Be sure to include the Form ID-40V payment voucher. The voucher should be included with your printed tax return. Here is a link to the Idaho.gov website where you can print the voucher from the website https://tax.idaho.gov/forms/EFO00090_04-26-2016.pdf.
