No street address to file extension form 7004 for form 1065 return

I am trying to mail in the form 7004 but USPS tools do not accept the following address:

Department of the Treasury
Internal Revenue Service Center
Ogden, UT 84201-0045

What is the street number or PO BOX for this ?
