I was in 2018 a member of an LLC in Indiana but I am a full time resident of Wisconsin and have received a K1 from the LLC, what Indiana form do I need to fill out?

This was a multi member LLC that is no longer in business.

Take a look at your Indiana IT-65 line 8. There should be something on that line.

Indiana requires that nonresident member's be included in a composite tax return that is filed with the Indiana partnership tax return. The LLC is required to remit the taxes on behalf of the nonresident member. As a result, if you have no other Indiana sourced income, then you have no additional filing requirement.
