How do I fill out a W-9 form for a husband and wife LLC in California and want to be a disregarded entity?

Who's name? both? Which tax classification box? Who's SSN, or use the business EIN?

The W-9 should be for the LLC business information, not either partner individually.  The election to report your income separately is only for tax purposes, so that you don't have to use a partnership tax return for the business.
