Form 1065 Schedule B 1, and 2a 2b

My spouse and I have a LLC. Our taxpreparer in the pass submitted us as Limited liability partnership. Last year I submitted as Limited Liability Company. Which is correct?

The taxpreparer also said yes to 2a and no to 2b. Last year I answered no to 2a and yes to 2b. Which is correct?

LLC is in Illinois. Not sure if this matters for these questions.
