Let me preface with, I no longer work for the company & have no interest in reaching out to them for clarification on the codes as everything about this company was messy & will take a week to get back to me with the info.
My employer entered two 'INS' values in box 14 on my W2. Unfortunately I do not know what either codes/values are referencing & its nearly impossible to know what codes to use intead, to prevent a duplicate entry. One is INS $334.20 & the other is INS $30.00 - how do I enter both of them when processing my taxes? I keep getting an error message but there are no other codes to select that seem relevant to my scenario.

Usually box 14 is informational but may sometimes affect your state return. The insurance on your w2 in box 14 will not have an effect on your state taxes so it is irrelevant for you.
CA does have an SDI deduction that CA companies withhold so that does not apply to you. As long as the other information is correct, wages, tax, etc then box 14 entries won't matter for you.
