Do I report taxes as a 1099 B or section 1256 contracts and straddles or form 988? I traded on Forex.com which is owned by Gain Capital Holdings Inc.

On Forex.com I used metatrader 4 trading platform and traded futures contracts. Also do I report this broker name as Forex.com or Gain Capital Holdings Inc.
