You will have to fill out  Form W-7, Application for IRS Individual Taxpayer Identification Number and have to print your tax return and mail both with proof of identity, and foreign status documents to:

Internal Revenue Service
Austin Service Center
ITIN Operation
P.O. Box 149342
Austin, TX 78714-9342

Can I do all of this on October if I file for a tax extension? I am not sure I can get the certified proof of identity and mail it before the 15th.

Yes, if you file an extension then you will have time until October 15 to file. But if you expect to owe money then extension won't give you extra time to pay your taxes, which are due on April 15, 2019 (June 15 if you're out of the country).

I don't expect to own them, I do donations plus I was in FMLA for two months. So I expect the opposite. Is June 15th when filling from the outside? I expect to be back in the US by April 18th.

Good,  then you do not have to worry about the paying part. Just file the extension and you will have plenty of time to get all the documents together and file the return by October 15th.

I am trying to file an extension through Turbo Tax but it won't allow me to leave my wife ITIN blank. What can be done about that?

You will have to print and mail the extension: <a rel="nofollow" target="_blank" href="https://www.irs.gov/pub/irs-pdf/f4868.pdf">https://www.irs.gov/pub/irs-pdf/f4868.pdf</a>  In the space that requests your spouse's Social Security Number, simply print the words "ITIN TO BE REQUESTED."
