When filing by mail, do I have to include the original copy of the W2 form and certificate of rent paid along with the printed form 1040EZ and form M1?

For the federal return that is mailed to the IRS, you would include a copy of the W-2 with the 1040EZ. 

Your question references Form M1, which is the Minnesota Department of Revenue Individual Tax Form. When mailing in your state return, attach Certificate of Rent Paid to Form M1.

For the Minnesota state return, the Minnesota Department of Revenue website indicates:

  • Behind your Form M1, assemble your Minnesota schedules according to the sequence number at the top of each. Place a copy of your federal return and schedules behind your Minnesota forms. Do not send in your W-2 or 1099 forms.
