Wage and Income Transcript via Form 4506-T

Hello! I am trying to get a Wage and Income Transcript ordered on behalf of my mother through the mail, and I am a bit confused on what boxes do you tick on Form 4506-T to let them know to send this. 

Also, since my parents file a joint return, how do I let them know to just send my mother's Wage and Income Transcript and not my father's? Do I also attach a piece of paper stating for the IRS to just send my mother's? Wage and Income Transcript? I will appreciate a response thank you.

Check box 8 for a wage and income transcript. Enter only your mother's name and Social Security number on lines 1a and 1b. Leave lines 2a and 2b blank.

I really need to get a copy of my Wage and Income Transcript for 2021.  Help.

@josaille Transcripts come from the IRS--not from TurboTax.

You can get a wage and income transcript from the IRS:  It will not show the actual W-2, 1099’s, etc, but will show the income information the IRS received.  It will not, however, show your state information. It will only show the federal information.
