In order to request a new card, you will need to contact Green Dot. Here are some steps that have helped others:
1. Call 1-888-285-4169.
2. You will be asked for your Card number or SSN.
3. You will be asked for your 6 digit Date of Birth.
4. When they ask your key selection select #5 and then #6 to speak with a live representative.
You can also go to their website visit Turbodebitcard.com and create an account.
