Yes you have to report all your income even if you don't get a tax form for it. Even $1 of bank interest. Were you an employee? They should send you a W2 for it. Or if you were an independent contractor or self employed they have to give you a 1099Misc if they pay you over $600.
i assume you got paid cash and no taxes taken out?

Hey thanks for the feedback! They sent me a w-2, but I'd moved and hadn't updated my address. Now I can't seem to get anyone on the phone in order to have them send it to my new address. I earned just over six hundred dollars and paid all the taxes - but don't have their EIN to report it on my return. I have all of the information of what I earned and what I paid in taxes, but do not have the w-2 and the necessary EIN.
Yes you need the actual W2. You didn't pay the taxes. You had withholding taken out. You need to enter it on your return. Even if you don't owe any more tax on it you might be able to get a refund of the withholding and EIC credit.
