Do not mail Form PA-8453 to the Pennsylvania Department of Revenue (PA DOR) unless you are notified by the PA DOR (very rare).
PA Form 8453 is required for all electronic returns that have not selected the Federal Self-Select PIN. Taxpayers filing electronically from home computers must keep the signed Form PA-8453 and supporting documents for three (3) years after the due date of the return, or the date the return was filed electronically, whichever date is later.

Where do I send PA signature form when E filing?
If you e-filed your return, you should not need to mail anything in unless the program instructed you to do so. Please see Do I need to mail anything after I've e-filed? for more information.
If you filed your taxes online with Turbo tax and printed or saved your return, you already have the form..It prints out with your return.It is PA 8453 and says:
PA Individual Income Tax Declaration for Electronic Filing at the top
