Lost card how to get a new one

i have called and gone the website i dont know my card number and the system is saying my social and date of birth dont match

This not working for me I called every number there is to get this issue fixed I can’t get a new card because it say my date of birth is wrong

When the phone asks you to enter information, choose lost or stolen, enter your social security number, and then don't enter anything else and it should eventually send you to a human for help.

Yes I lost this card that my money went on do to wrong address.  [addresses removed] my number is [phone number removed]
