I completed my return but haven't submitted it yet. After printing out my 1040 I realized that I forgot to include my middle initial. Do I need to go back and do this?

Yes, it would be best to go back and enter your middle initial.  

This will ensure that your name and social security number match what you used in prior years and also match social security records.
