How do i reorder a lost card

so i got the notice of my stimulus deposit but never received my replacement  card in  mail the address i gave them was different from what was on file so they decided to take it upon themselves and return it..... ups and my address is all correct i just need to speak with someone and verify address and so they caN REMAIL IT!!!

I CANT LOGON TO THE MOBILE APP FORGOT MY USERNAME AND WITHOUT THE NEW CARD I CANT RESET IT
