How do I find FORM R-10606 (worksheet for credit paid to another state) so I can download and print it?

I download my forms and selected "Include government and TurboTax worksheets" but R-10606 was not included.  I had earned income in my non resident state and unearned income in my resident state.  My resident state has asked me to mail in my R-10606 explaining how the credit for taxes paid to the non resident state were calculated.

Unfortunately, TurboTax does not support Louisiana's Form R-10606, Supplemental Worksheet for Credit for Taxes Paid to Other States for Forms IT-540 and IT-541.
