The IRS came out with the form just this month. Google the form and the IRS pdf of it will pop up.

I was sent a form not knowing I was not given the EIC! This has never happened to me before. I even had to pay this year for you all to do my taxes. How was this missed? I want a full refund from you. Now I have to wait 6-8 weeks to get the full refund I was owned. So aggravating!

It’s in the notice. It’s just the questions they ask. Google form 1511 you’ll be able to download it though
