Haven't filed Form 8843 for the past 4 years and planning to file them now. Do I need additional documentations with the form?

I didn't know about Form 8843 and haven't filed Form 8843 for the past 4 years, but I did file file tax return before and I was instructed to send in copies of my passport, visa and I-94 for the last date I entered the US. Do I need to do the same for Form 8843, or should I just sent it by itself? Is there any other documents I need to send with Form 8843?

Also, in form 8843 line 1a, they asked for "date you entered the United States". Is this the first date I entered the US, or the last date from now?
