Form 8955-ssa was filed about me? do I need to add it to my tax return?

I received a letter from a previous employer that said Form 8955-SSA was filed with the IRS regarding a retirement plan.

Do I add this to my tax return? If so, where?

This is informational only ... the plan administrator filed it on the retirement plan as required and a copy was sent to you as the administrator of the plan for your company for your records.
