For NQSO what is the Grant Date - is this the Award Date or Exercise Date? There is nothing on any of the forms from my brokerage firm that says Grant Date.

The Grant Date is the date your company gave you the stock option award - the Award Date.  More than likely, the options would have vested on a subsequent date.  The exercise date would be the date you turned your options in to stock or cash.
