I emailed my Colorado Tax return. I did not complete line 34. Turbo Tax did ask me to make entries. I sent the return to the State of Colorado.
As noted by @JulieS you can amend your CO state return (steps extracted below). Once you have started the amend process (step 7 below) you will need to navigate back to the "Other Situations" page (step 5 below) and find the "Sales Tax Refund" topic to claim the rebate.
