Why wouldn't Turbotax Premier file my Federal and State extensions?

I was forced to print and mail the Federal form 4868 extension instead of efiling it. I was forced to go to my state's website to file my state extension.

Trying to submit a return. Turbotax message says the submission is a duplicate. Then says they could not find a federal return that matches. Both submitted at SAME TIME.

If you are getting a federal return and there is an offset for the state. Will the fees for turbotax be deducted or withdrawn from my bank account

How do I indicate in Turbotax that I have a federal pensions, which is not subject to taxes in Hawaii, my state of residence?
