Who sends me the 1099-HC form

I have insurance through BCBS, CareFirst based on Washington DC but I live in MA. Where can I find the 1099-HC form with my subscriber number?

This form is provided by your health insurance carrier. To download an electronic copy of your Form 1099-HC, you may sign into your medical insurance carrier's online account, or contact their member services at the number on your ID card for more information.

You need to contact BCBS and request the form directly from them.

Why do you think you need that for your income tax return? The only health insurance form you are required to enter is a 1095A if you had insurance from the marketplace ("Obamacare").
