Where do I put survivor benefit.

My husband passed away April 19, 2024. He worked for the State of New York. I received a one time payment of $3,000 as a survivor benefit. Where do I put this amount.

I have a 1099-R for an annuity from a life insurance company that came to me upon my dad's death in March 2024. Is that considered survivor benefits for Ohio state taxes?
