Where do I enter Line 12 on Form 1099-DIV for Tax Exempt Dividents? Thank you

On the Form 1099-DIV for entering your dividend information click on the box labeled My form has info in other boxes (this is uncommon). Boxes 2b thru 16 will be displayed. Enter the tax exempt dividends in box 12.
