When submitting a 1040-x where should I send the form if mailing through usps from Ohio?

I think Filing Instructions may print with the 1040X, but if not, here's how to check at the IRS website by state.

If you plan to mail it through the United States Postal Service, go to the following page at the IRS website.  Scroll down and choose your state.   Then expand your state's section.   You will then see 2 columns--one for "if no payment is enclosed," and one for "if payment enclosed."   Choose the type of form in the list there and the relevant column's mailing address.
