What should a 1099-G form look like? Am I missing part of it?

I received a form 1099-G, it was sent to my parents house because I often travel so that's my permanent address. It was sent in the form of a card with perforated lines, upon opening it was a rather small and short card with the information inside.

 

However, it only states that it's the "copy b" and has one line of information. My social security and my refund total for the previous year. It has instructions stating that it's important tax info, but when I try to enter this form in turbotax it asks me a lot of questions about like what type of payment I received or whether it's corrected or not. I can't answer these questions because the card doesn't provide anymore info, so I'm thinking that it's possible that when I opened the card there was more info but I simply don't remember what it looked like or where I placed it. I've never had this type of form before and have no idea what to do with it.

A 1099G can be for many things.  Yours sounds like for the state refund you got last year.  Does say box 2 on it?  You are in the wrong spot.

 

To enter a state refund just type  1099-G (with the dash)  in the search box at the top of your return and click on Jump To it.

 

OR  Enter the 1099G under

 Federal on left

Wages & Income at top

 Scroll down to 1099-Misc and  Other Common Income

      You might need click on Show More to expand the section

 

Then it's the first line for Refunds Received for State/Local Tax Returns - Click Start

     You do not want the next line that says Other 1099G Income

 

On the first screen check YES - Received a State Refund.

 

But you might not need to enter it at all.  

A State Tax Refund is taxable if you itemized deductions on that prior year's federal return and took a deduction for state income taxes instead of the sale tax.  You got a deduction benefit for it so now you have to include it as income. If you took the standard deduction it is not taxable and you don't need to report it.

It lists tri-met self-employment, lane transit, statewide transit, state income, and then total tax. It has not other boxes and the one line listed is not numbered. So I may not have to report it at all? I don't remember "itemizing deductions on that prior year's federal return and taking a deduction for state income taxes instead of the sale tax," as you said.

Huh?  You first said it only lists 1 thing your refund total.  If you don't know what it's for as the the state.
