1099-G forms are issued by the State Government. If there is no information or you believe the information is not correct you should contact the state that issued it for verification.
If you received a 1099-G Form this year from a government agency, you may need to report some of the information it contains on your tax return. The most common uses of the 1099-G is to report:
Taxable unemployment compensation
Generally, you must include in taxable income any unemployment compensation from a state government.
State tax refunds
When you receive a refund, offset, or credit of state or local income tax, that amount appears in box 2 of the 1099-G form. However, you don't necessarily have to report this amount on your federal tax return or pay additional federal taxes.
You only need to report it as federal income if you took a federal deduction for paying those taxes in a prior year and that deduction actually reduced your federal taxes. Box 3 of the form will indicate the relevant tax year.
I have already filed taxes but just found I was never ask for my 1095g form from my TurboTax live pro. is it to late to add it?
It depends. Since you have already submitted your 2020 tax return electronically, you will have to wait until your tax return was either accepted or rejected.

I live in Iowa and used to commute to IL for work. Thus, I when I filed for unemployment, I had to file in the state of IL. I had federal and state taxes taken out, but my 1099G form doesn't specify which state was took out my taxes. IA or IL. Also, when I received by 1099G form, there's only one row of boxes. There's no 10, 10a, or 10b box. Is this normal for a 1099G form or do I do I need to contact the state of IL? I just don't want to get in trouble with the government. Thanks!
@Anonymous Since the unemployment is from Illinois, the state tax withheld would be paid in to that state.
As long as you can determine what is being reported on the 1099-G form so you make correct entries in the proper boxes in TurboTax, you can use the form you received to report your unemployment income.
