What form a 12 year old has to file for 2019 if he has unearned income below $2200, but higher than $1100, and parents do not want to put him in their tax return?

Is it just federal return or also state return that the child has to file?

Form 1040 will be created by Turbotax as you enter his income.  Be sure that you check the box saying that he can be claimed as a dependent.
