We are supposed to enter PFL Income Received From Insurance Company. This can't include PFL paid by the employer or any income reported on 1099-G. Where do we get this?

The amount should be reported on a tax statement issued by the insurance company. You will have to contact them and request the statement or amount you were paid.

Is the Employer Liability total include all the taxes and paid by Employer and NOT paid by employee - ie. Fed withholding? I'm using the ADP Payroll Taxes report.
