W2 box c Employer's name,address and zip code

Employer's name lists 3 lines before address, first line list CHC PAYROLL AGENT,INC second line is the name of the  ABC MEMORIAL HOS,INC  and the 3 line is the name again ABC MEMORIAL HOSP.   then the address. Turbo tax only as 2 lines for the name and then the address lines.
How should I enter this ? Should the payroll agent  be the first line?

It doesn't matter that much. IRS is relying on the EIN to identify the payer much more than the name. Go with the Payroll agent since it is the first name listed.
