Does 8.16% of your income applied to the entire year of coverage? What if you didn't have coverage for 3 months?

The 8.16 percent applies to the total of the employee’s household income for the year for purposes of an exemption from the individual mandate, it is not broken down into months.

If you do not have coverage for only three months, the penalty applies only to the three months, but the 8.16 applies to the whole year- which is unfair because if you were unemployed for example, for that month you had 0 income so it was unaffordable but the law was not written that way.

You can apply to the heathcare.org as a hardship exemption for the three months to get the exemption (under #14).
