Streamlined Filing Process

For the Streamlined Filing Process, you have to file 3 years of taxes and 6 years of FBAR. Do I still have to file 6 years of FBAR if I was above $10,000 for only 2 years?

Also, if I was filing now, would the 3 tax years be 2021, 2020, and 2019? Or would it be 2020, 2019, and 2018 (and then I just file 2021 separately)?
