Short Term Disability

I received NYS Short Term Disability and my employer's 3rd Party Short Term Disability Insurance while out for surgery. On my employer's W-2 there are 2 boxes that I have questions about. Is box 12J  my NYS Disability? I have not received a W-2 for this. Should I receive a separate w-2 for NYS Disability? Also, Box 14 is marked as 3rd Party Taxable Sick Pay, which is the other box in question. Is that the 3rd Party Sick pay from my employer's Short Term Disability Insurance? If so, I have an additional question.  I did get a W-2 for this from the insurance company. Do I include it as income even if it shows in box 14?

Short Term Disability

Yes, Box 12, code J represents your NYS disability wich is a nontaxable sick pay benefit.  Box 14 is an information box about your employers short term disability benefits paid. 

Since that information is on your W-2, it sounds like the income is already included on your employer's W-2.  However, since you also received a W-2 from the insurance company it would be a good idea to verify that the payments are included in your employer's W-2 before you finalize your return so that you don't duplicate that income.
