Why does my tax liability, tax paid, and Refund (or taxes due) not add up?

Of course, we in the Community cannot see your private tax data, but one common reason is that you have an entry on line 38 (Estimated tax penalty). It's not obvious, but the instructions on page 58 call for line 38 to be added to the entry on line 37. Is this it?

"Include any estimated tax penalty from line 38 in the amount you enter on line 37."

That wasn't the issue but I did find my mistake. It was adding the cost value I had in box 12 because I marked it as code A, instead of code DD. The box on my W-2 was 12a, which is where I confused it from.
