On form 1099G, is the "PAYER'S Federal Identification Number" also the State Identification Number? I don't have a box 10.

My 1099G form (state of maryland) only has boxes 1-5.  Box 1 is "Unemployment Copensation Paid".  Box 2 is "Repayment Amount". Box 3 is "Federal Tax Withheld". Box 4 is "State tax withheld" and box 5 is "Reemployment Trade Adjustment Assistance".  So Turbotax is asking for the amount in box 10a, which is actually my box 4.  Then for 10b, it wants my "state identification number".  Is that just the Maryland EIN?

That is NOT The IRS 1099-G that looks like this.

All I can suggest is that you put the values in the proper box according to the box label and not number.    However, if there was state tax withheld then the payers state ID number (that is not on your form) is required to properly credit the payment.

Of not state tax withheld in box 11 then leave all boxes, 10a, 10b & 11 BLANK.

Well that isn't going to work.  Because my 1099G looks different.  For example, Turbotax asked for the "federal tax withheld - from box 4".  Well, on my form, federal tax withheld is in box 3, and state withheld tax is in box 4.
