I need to contact a live cpa

good morning.....it takes me a bit to get messages and respond.....I AM being paid by ohio state medicaid waiver program and they do not take out taxes.....I AM trying to find out what and how to get this done so that I WILL not owe the irs plus, should i open an LLC since I AM an independant worker...?? please help and thank you in advance and for your prompt resonse.....

Contact them to see if any tax reporting form will be issued if any  ... ususally these payments are exempt from both  fed & state taxes.
