On the;
1. "File by Mail Instructions . . . " the last line says/shows "No Refund or Amount Due".
2. On page 1 of form 1040-X, line 16 shows $629, line 22 shows $0 "refunded to you".
3. On page 2 of form 1040-V (I have no page 1 of form 1040-V) is a payment voucher with $629.
I'm confused. Both lines 1 & 2 indicate $0 is to be refunded or due while the 1040-V shows the amount of payment as $629. What do I do?

When you amend your tax return, Form 1040-X shows your changes, shows the amount that was refunded or paid with the original return, and shows the additional amount due or refund.
Since line 16 shows $629, the 1040-X is saying that you paid the $629 at some point. If you did not, you still owe $629 for 2021.
The question is whether you have an amount due on line 20. Double check that and then update here with more details so that we can better assist you.

Columns "A" & "C" are the same.
I've been looking through personal accounts & at IRS.gov and can't find that any monies have been paid on 2021's return(s) . . .
. . . depressing I know so it looks like the $$ on the payment voucher is owed.
