Paid Family Leave & 401K via employer

Am I supposed to report paid family leave I received via my employer on my tax return? I'm uncertain but I think it should be on my W2, no? Also, I got a 401K via my employer, do I report that on my tax return? They only take out an amount of my paycheck & I did not withdraw anything. Thanks!

Paid Family Leave & 401K via employer

Your w2 should have both. 

  • Paid family leave should be included as part of your wages in box 1.
  •  Your 401k is denoted in box 12 with a D code.
