For NJ, do I add Cafe 125 HSA contributions to this form or am I double taxing myself? Does TurboTax already include my HSA contributions in my state return?

No, do not add your HSA contributions on this NJ screen.

Because TurboTax can identify HSA contributions on your W-2 (code W on box 12 of your W-2), this amount is automatically added back to NJ state income.
