I was put as a plaintiff into a class action lawsuit with a previous employer who I worked for in 2020. I didn't even know that I was a part of it, I just got a check and last year there was a check from the employer that I paid taxes on. I believe it was about labor law since the company wasn't paying the correct minimum wage for employees.
I got a tax form for the settlement award that has a 1099-MISC, 1099-INT and 2 W-2 forms. The first W-2 form has the address and EIN number from the place that sent me the check.
But the second W-2 form just has the name of the previous employer where I have not worked for more than a year, the state employers ID number and an item in Line 14 that says it is an SDI $5.50. It doesn't have a federal EIN number, address for the company or any wages.
How do I fill out that second form with no information? I put in what I had in it and it and Turbotax keeps saying to review and keeps pointing out the no EIN or address inputted.
Any help would be greatly appreciated.

If you itemize your deductions you can just add the Line 14 to the W2 from the Settlement Fund. If you do not itemize you can just disregard- this is just showing that your previous employer had to deduct and pay to the state the additional SDI.
Thank you for your reply. Since I do not itemize can I just delete that started form and keep the other W2 and 1099 forms?
Correct. In your circumstance the box 14 is informational only for you since you are taking the standard deduction and there is no income in boxes 1 and/or 16. Delete any entries for that form W2.
