On box 19 there is 2 amounts which one do i put down?

Box 19 of form W-2 is for Local Income Tax. If you have an amount there, you should also have a locality name in Box 20. Could the amount be a dollar amount and change? Or a dollar amount and the locality? Otherwise, let me know if it's another type of form.

Regardless of the type of form, there should only be one entry per box. If you have an error on a form, you need to discuss it with the person who issued it to you and they need to make the corrections.
