Non qualified annuity

Having trouble I have non qualified annuity 100,000 took early withdrawal 50,000 the company withheld 10% early withdrawal plus 3000 surrender charges I net 42,000

Got 1099

Box 1 gross says 42,000

Box 2 taxable blank

2a tax not determined is checked

Box 7 codes are D and 1

When I plug numbers in turbo tax say I gave to pay early withdrawal which I all ready did plus I been taxed on 42,000 as well I put that in already taxed

I called annuity up today again and still saying 1099 is correct

I m frustrated thanks have not been able to get resolved
