The restaurant my daughter worked for closed. No W-2 was sent out, and we don't know their EIN number. I tried to find the spot on the W-2 to select "Didn't get a W-2," but this check box is not found.
I tried to complete the 4852 form, but it won't let me change the selection from 1099-R to W-2. I removed the W-2 bit, and then the 4852 expects a W-2 to associate itself, too. I'm looking for any advice on my options. I tried the Feds, and they don't have any info for us. thank you, [PII Removed]
(I was able to create the 4852, but Turbo Tax is not allowing me to file as the EIN is missing).

The error you are getting is because you are completing the form 4852 in the wrong section of TurboTax. You will need to go into the program as if you were entering a W2. Follow these steps to complete your form:
You will have to file a paper return.
Hi, thx for the info.
I have done all of what you mentioned. However, I was stuck filing my returns because TT still expected the EIN to be entered. It would be nice if TT would inform you of mailing in the tax returns if the 4852 were used.
So, I need to mail in both returns because of a lazy restaurant owner. Do I send in the 4852 form and the W-2 with the Fed and State returns?
Try using 99-0999999 to see if you can efile the return? This has worked with missing EIN.
Thanks. I did try 00-000000, and I will try 99-999999. But I might still need to file both returns via US Mail, as Form 4852 might not be sent via online TT submission.
Try also 99-0999999 if all nines don't work.
