The 1095-C confirms an offer of coverage. If you had health insurance, you just check off that you did. The 1095-C form doesn't get entered anywhere.
Go to Federal Taxes- Health Insurance to report that you were covered by insurance.
The 5498-SA just verifies an HSA contribution. If you made contributions through your employer, that's already accounted for via the Code W in Box 12 of your W2. A 5498-SA is also not literally keyed in anywhere.
