You may not have any income to report.

If this is from California EDD, unless you were previously receiving unemployment and became disabled during that period, this isn't taxable on either your federal or state income taxes.

"SDI benefits are taxable only if paid as a substitute for unemployment insurance (UI) benefits. This could occur if a person was receiving UI benefits and then became disabled. When SDI benefits are received as a substitute for UI benefits, the SDI is taxable by the federal government but is not taxable by the State of California.""

If any part of your payments are taxable, the state of California will issue a 1099-G. http://www.taxes.ca.gov/Income_Tax/specialind.shtml#Disability

If this is from a different kind of disability plan, if you pay the entire cost of a sickness or injury plan with after-tax money, you do not need to report any payments you receive under the plan as income.

How do I get the 1099 form from my disability insurance benefits.  Can I file taxes with out this form?

If your disability benefits are taxable, you will receive a form. Please click on this link regarding Short Term Disability to help determine your situation. You can call and ask if you will receive a form. You should not file your taxes until you know if you should include the disability income.
