Name not recognized

When I’m ready to file my tax returns, I attempt to complete the consent form for MAX BENEFITS which I purchased. I’m instructed to sign form using name below but when I input the exact name shown, I get an error message and it’s not excepted. The date and email address on form are good when entered but not the names of myself and wife.
